Matthieu Legrand, MD, PhD, is a Tenured Professor in the Department of Anesthesia and Perioperative Medicine and the Division of Critical Care Medicine at the University of California, San Francisco. He serves as Director of Clinical Research, with responsibility for advancing clinical research infrastructure and supporting investigator-initiated and multicenter studies.

Dr. Legrand’s research focuses on the mechanisms and treatment of sepsis, shock, and perioperative complications, with particular emphasis on acute kidney injury, cardiovascular dysfunction, and vasopressor therapy. A central pillar of his work is the application of biomarkers and multi-omic data to improve diagnosis, risk stratification, and prognostication in critically ill patients. He has developed and implemented biomarker-based and subphenotype-driven strategies to enrich clinical trials and enable more precise, patient-centered therapeutic evaluation. He has led and co-led multiple large randomized clinical trials and observational cohorts using innovative designs, including cluster-randomized crossover trials and Bayesian analytical approaches. His research program emphasizes heterogeneity of treatment effect, causal inference, and the integration of advanced analytics to optimize trial design and interpretation. In parallel, Dr. Legrand is deeply engaged in mentoring fellows, residents, and junior faculty, and in fostering multidisciplinary collaboration across anesthesia, critical care, and perioperative medicine. His overarching goal is to translate mechanistic and biomarker-informed insights into scalable interventions that improve outcomes in critically ill and surgical populations.
