Subsequently, he completed the Robert Wood Johnson Clinical Scholars Program at the University of Washington. Dr. Kohlwes returned to UCSF to assume the directorship of the PRIME program at the San Francisco VAMC. During his tenure as Director, the PRIME program has expanded to include the majority of the UCSF categorical residents each year becoming the largest residency track at UCSF. He has published several manuscripts on the PRIME Program, which focuses on providing an immersion clinical research experience for internal medicine residents. Currently, two thirds of PRIME residents publish their residency research projects with over 300 first-author publications over the last 12 years.

His position as an Associate Editor for Clinical Material for the Journal of General Internal Medicine has led to the merging of his interests in clinical care and evidence based medicine. Working with a team of editors he created the Exercises in Clinical Reasoning series. This series is expanding into a national online teaching tool for Clinical Reasoning. His interest in evidence based medicine and clinical practice have led him to do his Updates in General Internal Medicine for UCSF conferences and medical grand rounds annually since 2004.
